Evidence of Insurability

(also known as Proof of Good Health)

Newly hired employees may enroll without proof of good health — also called evidence of insurability (EOI) — during their enrollment eligibility period, which is 30 days from their date of hire. Employees may select from the following supplemental life coverage:

  • for yourself — up to three times your Metro salary, in increments of $10,000, to a maximum of $200,000
  • for your spouse — $10,000
  • for each child — $5,000

In future enrollments, proof of good health will be required for supplemental life, both to enroll and to buy coverage higher than the levels listed above. For more information, contact the insurance carrier.

NOTE: If you do not enroll in supplemental life insurance when it is first available to you, you may not be able to enroll later if you have certain medical conditions. After initial enrollment, you may not qualify for coverage if you fail to meet the insurance carrier’s medical guidelines.
