Dear Davidson County Voter:
Following the Davidson County Election Commission’s notification to you that the theft of a laptop resulted in a breach of security of your personal information, the Metropolitan Government of Nashville contracted with Debix Identity Protection Network. The contract with Debix allows you to register for a full year of its identity protection service at no cost to you. If you are interested in this service, you must register by April 16, 2008.
The Metropolitan Government of Nashville has no information to indicate that any data from the stolen laptop has been accessed or that any taxpayer information has been misused. The contract with Debix will provide you with protection in the event that your personal information is misused.
Davidson County voters may register with Debix in one of two ways: online at www.debix.com/nashville or by mail using the attached form. The service is valid for one year from the date of registration. You will need to provide the activation code located at the top right of this letter when registering. Please note that part of the sign-up process includes receiving a telephone call from Debix soon after you initiate the registration process.
As part of the Debix service, you will receive an immediate warning call every time someone applies for credit in your name, fraud alerts at the three national credit bureaus, $10,000 of identity protection insurance, and placement on the National Do Not Call Registry if requested. If you have further questions about Debix or its coverage, please contact them directly at 1(888) 332-4963 (toll free) between 7:00 am and 7:00 pm weekdays.
